Participant Waiver and Release of Liability
I understand and acknowledge that participation in the ice cream eating contest involves certain risks, including but not limited to the possibility of accidental injury. I agree that neither Turtle Roll Homemade Ice Cream & Shaved Ice, Turtle Roll LLC, nor any of its owners, employees, volunteers, or affiliates shall be held responsible for any injuries, medical, dental, or other expenses incurred as a result of my participation in this activity.
By signing below, I voluntarily release and hold harmless Turtle Roll LLC and all associated parties from any and all liability for any injuries or damages that may arise from participation in this event.
Minor Participant Waiver and Parental Consent
As the parent or legal guardian of the contestant, I give my full consent and permission for my child to participate in the ice cream eating contest hosted by Turtle Roll Homemade Ice Cream & Shaved Ice.
I understand that participation involves inherent risks and agree that Turtle Roll Homemade Ice Cream & Shaved Ice, Turtle Roll LLC, and all associated parties shall not be held liable for any injuries, medical, dental, or other expenses resulting from participation in the event.
I hereby release and hold harmless Turtle Roll LLC and its representatives from any and all claims of liability arising out of or related to my child’s participation.
